00:00:00Thanks to our guests, María Teresa Cardona, Israel Ortega,
00:00:03Andrés Flores and Octavio Pescador, and to all of you for allowing us to enter
00:00:07your homes. I am Juan Carlos López, on behalf of the technical
00:00:11and production team of CNN Washington, of clash of opinions from our colleagues in
00:00:20live on USA and next Sunday here on Choque de opiniones de
00:00:24CNE.
00:00:41We are CNN, the world leader in
00:00:45was a protest and it's really one of news.
00:00:49The Full Story with Anderson Cooper. I'm Anderson Cooper, welcome to the Full
00:00:53Story. While the Trump administration has tightened measures against
00:00:56undocumented immigrants in the United States, it has also taken initiatives to limit
00:01:00legal immigration, arguing, in part, to preserve jobs for
00:01:04American workers. However, hospitals across the country are increasingly
00:01:08turning to immigrant labor to fill positions for doctors, nurses,
00:01:12and technicians. The healthcare sector has been facing a staff shortage for over a decade,
00:01:16a shortage that became even more severe after the
00:01:19COVID-19 pandemic. The situation is especially critical in rural areas
00:01:22where hospitals struggle to attract candidates. One such facility is
00:01:26West River Hospital in Hettinger, North Dakota. It is
00:01:30located near South Dakota and Montana and provides medical services to all
00:01:33three states. More than 13% of its full-time staff come from
00:01:37other countries and fill positions that in some cases had been vacant
00:01:41for years. Edla Bandera of CNN and her team spent 30
00:01:45consecutive days filming inside this hospital, accompanying doctors and
00:01:49nurses from the emergency room to the operating rooms to show you the reality of
00:01:52life in a small town hospital and how much they depend on
00:01:56immigrant labor, both the hospital and the patients you will see in the next hour,
00:02:00granted permission to be
00:02:01filmed.
00:02:10Do you think we have understood the challenges they face? I
00:02:14hope so, no answer, ma'am, no
00:02:17answer, we'll probably
00:02:21let her go. Father Adam is here. Push,
00:02:24push. Good, you're working
00:02:27hard, you can already see the head.
00:02:31All right, preparing some fluids, let's get started. Tonight when she came
00:02:34home she was vomiting, I'll check her blood to make sure there's no stress
00:02:37on her heart, she needs oxygen, she had
00:02:41abdominal pain, it got worse, her surgery
00:02:44is over, we're just waiting for her to
00:02:46wake up, you're at a very high risk of having a heart
00:02:49attack, we're going to check the baby's
00:02:53movements, it's coming, yes, it's
00:02:55coming,
00:02:58country, left at 9:30 in everything seems to be fine, no reason to worry. 30 days from the inside, hospitals and the
00:03:02immigrant workforce,
00:03:02inmigrante,
00:03:09you're listening to KNDC 1490
00:03:12AM 106.7 FM in
00:03:14Hettinger,
00:03:18I'm going to retire in about 2
00:03:19years,
00:03:22I'm worried about someone coming to take my
00:03:31I don't know if they're going to find someone to fill my
00:03:34position,
00:03:41all rural areas are running out of people,
00:03:44it's becoming increasingly difficult to recruit and retain staff to work in the
00:03:48healthcare sector, and the situation worsened after
00:03:52COVID-19,
00:03:58in Hettinger live.
00:04:02about 100 people.
00:04:05We are 112 km from Montana,
00:04:096 km from South Dakota. But our
00:04:12service area at the
00:04:13hospital
00:04:19covers about 52,000 square kilometers.
00:04:23We serve 2,000 people in a large
00:04:26area.
00:04:30that offers similar services is 112 km from
00:04:34here and there are already people who drive 145 km to
00:04:38get
00:04:38here.
00:04:47We all perform multiple functions, the executive
00:04:50director drove the
00:04:51ambulance.
00:04:55It's difficult to recruit staff because it requires a lot of training to be a technician.
00:05:02of emergency medical services, hello Paul, I'm Dr. Hull, what happened to
00:05:06you today, did you feel dizzy, chest pain, difficulty breathing, what happened,
00:05:10I've been doing ambulance transfers for three years. as a driver,
00:05:13she also has emphysema and asthma, helping to transport
00:05:17people,
00:05:22did they do an electrocardiogram on you,
00:05:25we currently have more than 40 vacant positions, this is my
00:05:28office with about 200 employees currently, this used to be the
00:05:32lobby,
00:05:35this position has been vacant for about 10 years since
00:05:382016.
00:05:46The drive to Hettinger, North Dakota is beautiful, I'm going south,
00:05:50sur,
00:05:53the views are
00:05:54majestic,
00:05:59vast
00:06:00plains,
00:06:03the landscape is
00:06:04breathtaking.
00:06:14How do you pronounce the name of the town? Hettinger.
00:06:17See the
00:06:20map.
00:06:24You feel like you're in the middle of nowhere. A
00:06:27little bit and that's good. That's the
00:06:30goal.
00:06:37And also from
00:06:38Montana.
00:06:42This small hospital serves three states, three states,
00:06:53what challenges do you face running a small hospital in a small
00:06:56town? We have the resources for the
00:07:00materials and
00:07:01tools,
00:07:04but we lack the workforce, Hettinger Hospital, how can I
00:07:08help you?
00:07:12We have a lady coming in by ambulance, they found her unconscious at her
00:07:16home.
00:07:19No response, ma'am, no
00:07:21response.
00:07:25We'll probably admit her, keep her comfortable and let her
00:07:27die. They called her
00:07:30priest. Father Adam is
00:07:34here. The census information on the demographics
00:07:37of the patients you treat shows that 90% are
00:07:40white. It was probably
00:07:4398%. Not a big cut.
00:07:46We have a very large elderly population. I think it's deep
00:07:50and every time you use your hand it will open wider. Maybe insert one to bring
00:07:54that deep tissue together. We have our large
00:07:57farming population.
00:08:00That is, farmers, ranchers. Good thing
00:08:03your doctor had you get the tetanus shot. There are
00:08:07fewer young people, but
00:08:16pain. Take a deep breath, inhale and exhale. It's a
00:08:20struggle because of hiring. Does
00:08:24that hurt? We are very far from everything and we must do what
00:09:02Shower's funeral, what shoes should I wear? I
00:09:05think Dr. Shower came here after finishing his
00:09:08residency,
00:09:11he was a very important part of this place for many
00:09:15years, he was really an advocate for
00:09:19rural healthcare,
00:09:21well, I think this funeral will be
00:09:23big,
00:09:29hello, how are you all, it's been a long time since
00:09:32I've seen you, I have to sit in the aisle because
00:09:36I'm on call, but maybe I'll sit on the side behind
00:09:39Jerry, yes.
00:10:55who will retire,
00:10:57but it is becoming increasingly difficult to find people to fill the
00:11:01vacancies, there are simply fewer and fewer people willing to come
00:11:04here and engage in this type of
00:11:13operating room
00:11:58Today I was home making
00:12:00lunch,
00:12:03then they called me and told me I had a patient for a blood
00:12:07draw in emergency room
00:12:092, I'm from the
00:12:12Philippines, I grew up in a town similar to
00:12:16this one,
00:12:20it's like I never left
00:12:22home.
00:12:25give them the immigrant labor that is coming in to keep this place
00:12:28running, keep squeezing my hand if you feel any discomfort, that's becoming
00:12:32an increasingly important part of our survival, it's going
00:12:36to move, it's slowing down, this week, on Thursday
00:12:39we have a person coming in for an interview for an internal
00:13:10comprehensive analysis of how many applications I
00:13:13received, 31 from the Philippines, 11 from
00:13:17Nigeria, and three from
00:13:18Ghana.
00:14:26age. Many say they want to reach Mexico
00:14:29City, but others seek to go further
00:14:32north.
00:14:36The Mexican army's anti-drone battalion consists of
00:14:39these types of inhibitor elements to bring down drones
00:14:43that have become a threat. Chiapas
00:14:46authorities have detected a cattle smuggling
00:14:49point. This is one of the illegal crossing
00:14:53points for cattle going from Central America to
00:14:56Mexico. Thousands of protesters, workers from this
00:14:59country, left at 9:30 in the morning from the
00:15:03Angel of Independence. I am in the municipality of
00:15:06Teuchitlán in the Mexican state of Jalisco. Here
00:15:09the discovery was macabre by collectives of people
00:15:13searching for disappeared persons. What made
00:15:15you come here with the suspicion that your son might be on this
00:15:19ranch?
00:16:33I'm Gabi Frías in Mexico City, we are CNN.
00:16:40The full story with Anderson
00:16:41Cooper.
00:16:47Okay, let's jump right in.
00:16:51He's an 80-year-old
00:16:52man.
00:16:55He came in because he has chest pain. He's at a pretty high
00:16:59risk of having a heart attack right now and it might be
00:17:01developing.
00:17:07We're going to do a blood
00:17:08test for him,
00:17:15how important is this hospital to you? It's very
00:17:19important, it's the closest
00:17:22to us, there are only small
00:17:25towns around,
00:17:31I analyzed the blood samples from the emergency patient, this is a
00:17:35hematology machine. How long have you been working
00:17:38here, since 2022, 4
00:17:42years, we're all Filipino
00:17:46here,
00:17:49the
00:17:53migration flow is, oh, that would seem to be, it's really the
00:17:56most viable way for me to have a suitable candidate to work
00:17:58here.
00:18:11We received 31 applications directly online
00:18:14from the Philippines, all from very
00:18:17qualified candidates, we had 11 applicants from Nigeria and
00:18:21three from Ghana. We also had two very viable
00:18:25candidates, both American. We
00:18:29offered the job to both
00:18:30of them,
00:18:33but both declined.
00:19:24university.
00:19:31It's for the
00:19:32salary.
00:19:37amount of
00:20:43Why did you come to the United States to work?
00:20:47It's my dream.
00:21:07are they fasting,
00:21:09yes, we only had about three applicants between 2022 and
00:21:132024, two were from the United
00:21:17States, she drank water.
00:21:33not qualified, Marichu was the applicant,
00:21:49to fill the
00:22:16we were both born and raised here.
00:22:38as you do, this photo is just before our
00:22:4250th anniversary, it gives you peace of mind to know that hospital
00:22:46is nearby,
00:22:48yes, nearby a... got kicked in the head by a horse, a
00:22:52man got off his combine and broke his leg, it's
00:22:55necessary, this is an old one, when we had
00:22:59our children, we went there, if we had gone to Bowman, our
00:23:03children would not have made it, because they were born prematurely, what do you
00:23:07mean by that, they would have died,
00:23:10and when your children were born, there were more American-born employees
00:23:14at the hospital, have you seen that change, yes, over the
00:23:18years.
00:23:50improve your life, oh yes, of
00:23:52course,
00:23:54in what way, many things, money,
00:23:58time,
00:23:58opportunities,
00:24:03before Ursula and Melissa arrived, the positions
00:24:06were vacant for at least 3 and a half
00:24:10years, nobody
00:24:11applied,
00:24:19we took
00:24:22it out. and now we're going to wake up the patient,
00:24:26I'll hold your hand because we don't want you to scratch your eyes,
00:24:30I don't know what the girl who woke me up was called,
00:24:34she's still in her fantasy world, Leona, she said wake
00:24:37up, you'll go back to sleep, the surgery is
00:24:41over, we're waiting for you to wake up, why did
00:24:44you want to work in the medical field, you're in the recovery
00:24:48room. I like to help
00:24:52people, for me it's a great satisfaction
00:24:56to share my knowledge with someone and know that it will be
00:24:59useful to them,
00:25:02here we had twins 49 years ago, I don't
00:25:05plan to return to our
00:25:07country,
00:25:10right now it seems a little hopeless
00:25:12to go back, you want to be an American
00:25:16citizen,
00:25:22there are many people who say that there are immigrants who come to this country, they take jobs
00:25:26from Americans, have you ever heard that or thought about
00:25:29that? We cannot live next to each other
00:25:33without taking anything from each other, how much do you appreciate the immigrants who work
00:25:37here. We need them, that's all I can
00:25:40say. Do you think that's a shared
00:25:43sentiment by everyone who lives in the
00:25:46area? Not necessarily.
00:25:53people who still say, I don't think they should be here, did you really hear that, when you hear
00:25:56people say what you're telling me, do you say anything to them, yes, I
00:26:00tell them, they're doing their job, be grateful to
00:26:03have them, otherwise, you might not even be
00:26:07alive without those people helping
00:26:08you.
00:26:16One reason we are here in Hettinger, has to do
00:26:19with something I've been reporting on for the last
00:26:22year.
00:26:26In Texas, tension has arisen in the Dallas suburbs.
00:26:30This is a town
00:26:33council meeting, most people are probably retired and don't understand how the H1B visa
00:26:37takes jobs from Americans, we need to
00:26:40take back the American worker and support our doctors, not give away
00:26:44our country. This
00:26:48the most overtly racist things I've seen in a long
00:26:50time.
00:26:57This evokes what we heard in Charlottesville a few years ago.
00:27:01Much of this dispute focuses on the number of immigrants who have arrived in the
00:27:05region on H1B visas,
00:27:08that's why it's important to be in a place like
00:27:10Hettinger. You come from a broken country
00:27:13and you want to bring that culture here, it
00:27:17represents us, the full story with Anderson Cooper.
00:28:22The need to migrate knows no age, many
00:28:26say they want to reach Mexico City, but others seek
00:28:30to go further north.
00:28:34The Mexican army's anti-drone battalion consists of
00:28:38these types of inhibitor elements to bring down drones
00:28:41that have become a threat. Chiapas
00:28:44authorities have detected a cattle smuggling
00:28:48point. This is one of the illegal crossing
00:28:51points for cattle going from Central America to
00:28:55Mexico. Thousands of protesters, workers from this
00:29:00the morning from the Angel of Independence, I am in the municipality
00:29:04of Teuchitlán, in the Mexican state of Jalisco, here
00:29:08the discovery was macabre by collectives of people
00:29:11searching for disappeared persons. What made
00:29:14you come here with the suspicion that your son might be on this ranch?
00:30:51I was born in that hospital. You welcome these
00:30:55immigrant workers who come and it's fine. We need
00:30:59people to cover it.
